The purpose of this document is to illustrate how to complete the plan and what can be included in each section only. Support plans are highly specific to each child and their individual needs and so this plan is not suitable to use for another child. The review section is completed to illustrate how it might be done, however it would be blank at the beginning of the year when the plan is first prepared.
Also included in this download is a comprehensive guide on how to complete a support plan and to identify suitable learning targets. The same template is used, and general process is followed, for support plans at all 3 levels of the continuum of support (Classroom Support/ School Support/ School Support +), although a SSP+ plan will have the most detail as it is the highest level on the Continuum of Support. Examples of significant care needs (in addition to those included in this sample plan) which can be included in the care support planning section of the student support file, can be found in the NCSE’s Toolkit.
