Integrated Student Support and Care Needs Plan

This is an example of a completed School Support Plus and Additional Care Support Plan for a fictional student in Junior Infants. The plan is for a student who requires both SET and SNA support at the highest level of the Continuum of Support.

Planning for the student’s learning and care needs is incorporated into one Student Support Plan within the Student Support File, rather than using a School Support Plan for learning needs alongside a separate Personal Pupil Plan for care needs. This integrated approach was set out by the National Council for Special Education (NCSE) in its 2024 Toolkit on the Deployment of SNA Resources.

Department of Education and Youth Circular 0032/2026 on The Role of Special Needs Assistants in Recognised Primary, Post-Primary and Special Schools, has superseded Circular 0030/2014, under which schools were required to prepare a Personal Pupil Plan for students receiving SNA support. The new circular identifies the Student Support Plan as the student’s individualised learning and care plan.

The purpose of this completed example is to illustrate how the plan may be completed and the type of information that may be included in each section. Support plans must be based on the individual student’s identified strengths, interests and needs. This example should not be copied or used as a plan for another student. The review sections have been completed to demonstrate how a review might be recorded; these sections would normally be left blank when the plan is first prepared.

Also included in this download is a comprehensive guide to completing a Student Support Plan and identifying appropriate learning targets. The same template and general problem-solving process are used for support planning at all three levels of the Continuum of Support: Classroom Support, School Support and School Support Plus. A School Support Plus plan will generally contain the greatest level of detail, as it provides for students requiring individualised and intensive support.
