School Support Plus Plan Example – Level 3 Continuum of Support

This is an example of a completed School Support Plus plan which is a long term yearly support plan for pupils who receive support at the highest level (level 3) of the continuum of support. This plan follows the NEPS template and will serve as a useful example for teachers who are new to SET. Following the introduction of the new SET allocation model in 2017, this “School Support File” is the recommended long term support planning template for schools to use.
This plan is based on a Third Class child with suspected Autism (a diagnosis is no longer needed for a child to be in receipt of the highest level of support).
Also included in this download is a comprehensive guide on how to complete a support plan and to identifying suitable targets.
The purpose of this document is to illustrate how to complete the plan what can be included in each section. Support plans are highly specific to each child and their individual needs and so this plan is not suitable to use for another child.
