This completed School Support Plus (SSP+) Plan example has been created using the official template recommended in the 2024 NCSE Toolkit for the Deployment of SNA Resources. Designed for Irish primary schools, it provides a fully filled-in individual pupil support plan for a fictional child in an autism class with significant needs, including autism and a moderate general learning disability.
The example demonstrates how to complete every section of the Student Support File, including the Support Checklist, Support Plan, Care Needs Plan, Log of Actions, and Review records. It also integrates realistic care needs targets (toileting, hygiene, mealtime support, emotional regulation, and independence) alongside SMART learning targets (communication, literacy, numeracy, and social/emotional development).
This resource is especially useful for autism class teachers and SNAs who want to see what a high-quality School Support Plus Plan should look like in practice. It illustrates how to:
-
Incorporate input from parents, SNAs, SLT, OT, and NEPS into planning.
-
Write clear, evidence-informed SMART targets for pupils with complex needs.
-
Document care needs in line with the NCSE SNA Deployment Toolkit.
-
Demonstrate best practice for reviews and year-long planning cycles.
By showing a realistic, detailed example, this resource saves teachers time, reduces stress, and ensures compliance with Department of Education expectations while supporting better outcomes for pupils in special classes.
Included with the plan is a step-by-step guide to writing support plans, so you can confidently create and adapt plans for your own pupils. Teachers will gain clarity on the difference between the Student Support File and the Support Plan, understand how the process applies at Classroom, School Support, and School Support Plus levels, and see how to keep records that meet Department of Education expectations.
